Elation Electronic 健康 Record (EHR)

The 学生 健康 Clinic's medical practice has transitioned to an Electronic 健康 Record (EHR). All medical patients seen at the 学生 健康 Clinic will need to create an Elation Electronic 健康 Record account.

Once an appointment has been scheduled, you will receive an email to your Hawkeye email address with an account entry link to "Elation EHR".

Once an account has been created in Elation Electronic 健康 Record (EHR), students can:

  • Complete demographics, medical history intake questions, health screening questionnaires, 和同意.
  • Access their ElationPassport and view their medical records including their office visits, 药物, 治疗方案, and educational handouts.

All 健康 Sciences program students and RedTail Athletes will have an Electronic 健康 Record.

If campus is closed, the 学生 健康 Clinic is also closed. This includes weather-related closings. See scheduled college closings.

任命 are required. Please call or email to schedule an appointment.

The Nurse Practitioner and School Counselor are also available via Tele 健康.
